Office Policies
Appointment Policy
We are dedicated to staying on schedule and seeing all of our patients at the time of their appointment. Please note that we reschedule any appointment if the patient is more than 10 minutes late. Being that we reserve time especially for you, if you need to reschedule an appointment, we require 48 hours’ notice. For the first missed appointment or short notification of less than 48 hours’ notice, a $75 fee will automatically be charged to your account, $100 for the second and anything after that will result in the full appointment charge. Moving forward, a deposit will be collected to schedule future appointments and will remain on account if there is an appointment on schedule, or it will be forfeited due to a failed appointment.
Financial Policy
Our office accepts all major credit cards, cash, check and Care Credit. All co-payments are due in full at the time of service. A fee of $25 will be charged for any returned checks due to insufficient funds. In-office 3-month payment plans must be established prior to the day of treatment with appropriate forms signed and credit card retained for payments to process. We accept Care Credit payment for treatment over $300 for 6-month interests free or minimum of $2,400 for 12-months, senior discounts will not be applied. A financial charge of 1.5% will apply after two statements have been sent and after four months of statements account will be turned over to our collection agency.

Insurance Policy
We can bill all PPO insurance plans and will gladly bill your insurance for you. Please note that not all insurance companies will reimburse payment to our office, rather you the patient, if this is the case with your individual plan, full treatment balance is due at time of service. However, we remind you that billing your insurance is a courtesy to you from our office. We will actively work with your insurance for payment for up to 3 months past the treatment date, we will then ask for your assistance in coordinating with your insurance company for reimbursement. Your insurance policy is a contract between you, your employer and your insurance company; we cannot make any guarantee of any estimates we provide you, though we will do our best to see that you receive your maximum benefits. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated.
Standard Of Care Policy
Our primary concern is your complete oral health. We strive to provide all of our patients with the best standard of care possible. We require x-rays prior to your first prophylaxis (cleaning) and recommend checkup radiographs once a year and full mouth radiographs every 5 years. It is the returning patient’s option to decline radiographs, though only for a period of 24 months. Warranty of work will be contingent upon maintaining continued preventive care with our office.
